Episode 03: Space Shuttle Columbia
The Columbia space shuttle exploded over the Texas desert on 1 February 2003.
It was the second catastrophic loss in the space shuttle programme.
When the newspapers covered the story the next day, some attributed blame to the age of the shuttle, which had first flown 22 years earlier.
But those within Nasa knew better, and some had warned against the course of action that led to its tragic demise.
